- Receipts for advance payments of personal income tax
- Passport with mark(s) on the crossing of the state border of the Republic of Kazakhstan in the last 12 months
- Medical certificate (form 028/y, issued within the last 12 months)
- Medical insurance covering basic and specialized medical care for at least the period specified in the application
- Agreement on the intention to conclude an employment contract with an employer in Kazakhstan (for initial issuance)
- Employment contract with an employer in Kazakhstan (for extension)
