• Australian/overseas police certificates for last 10 years since age 16 (if applicable)
• Military service records or discharge papers if applicable
• Form 80
• Form 1221
• Proof of competent English (for you, partner, dependants)
• Partner evidence (identity documents, photos, marriage certificate or relationship registration, evidence of genuine relationship)
• Dependants under 18: birth certificates, adoption papers, parental consent (Form 1229/statutory declaration/court order)
• Dependants over 18: identity, relationship, Form 47a, financial dependency, medical report if over 23 & disabled
• Skills assessment documents (occupation/registration/admission to practice)
• Expression of Interest supporting documents as per points table
• Translations of non-English documents
