• Evidence of identity (e.g. 3 documents showing birth name, date of birth, gender, photograph, signature, and current address)
• Evidence of any change of name
• Full birth certificate showing parents' names or family/household registration document
• Official marriage/divorce/change of name/adoption documents as applicable
• Photograph and signature document (e.g. passport, driver licence, national ID card)
• Evidence of residential address (e.g. utility bill, rental contract)
• Form 1195 Identity Declaration with signed photograph
• Good character documents if over 18
• Overseas penal clearance certificates if required per time spent/locations
• Australian National Police Check (done by department for those 16 and over)
• Evidence of how citizenship ceased (e.g. prior citizenship certificate, loss documentation, foreign naturalisation certificate)
• Translation for non-English documents with translator’s details
• All documents certified as required
