- Evidence of identity (from birth to present, including birth name, date of birth, gender, photograph, signature, address)
- Official evidence of any change of name (marriage or divorce certificate, change of name documents)
- Photograph and signature document (passport, driver licence, national identity card, etc.)
- Evidence of residential address (utility bill, rental contract, bank statement)
- Good character documents (police certificates, overseas penal clearance certificates, as applicable)
- Supporting documents for exemptions, ministerial discretions, concessions if claimed
- Evidence of eligibility (e.g. for incapacity, loss of citizenship, fee concession)
- Documents relating to children included in the application (full birth certificate showing parent's details, passport, proof of parental responsibility if applicable)
