Documents
- Passport: current passport showing photo, personal details, issue and expiry dates
- National identity card (if you have one)
- Proof of change of name (if applicable): marriage/divorce certificate, change of name documents, documents showing other names used
- Medical treatment documents: letter from treating doctor in Australia detailing medical condition, required treatment, estimated timeframe and cost, and (if required) Form 1507 Evidence of intended medical treatment
- Birth certificate (if applicant is under 18): showing both parents’ names OR alternative ID documents (family book, government-issued ID, court documents, or family census)
- Legal guardianship proof (if not with both biological parents)
- Consent form for minors: Form 1229 or a statutory declaration from anyone with legal rights not traveling with applicant
- Photo/signature ID of consent-giving person (passport/driver’s license)
- Undertaking declaration for minors not staying with a relative/guardian: Form 1257
- Financial documents: proof of access to enough money for treatment and support, letters from any supporting Australians, hospital letter confirming payment arrangements (public hospital letter must be from financial officer/administration and confirm no disadvantage to Australian access)
- Police certificate (if requested)
- English translations of documents if not in English (originals and translations)
