• Current passport pages (photo, personal details, issue and expiry)
• National identity card, if you have one
• Proof of change of name, if relevant
• Record of achievement documents (awards, publications, employment, recognitions as specified)
• Completed Nomination for National Innovation visa (Form 1000)
• Evidence of nominator's national reputation and status (citizen, PR, NZ citizen, or Australian organisation)
• Proof of ability to establish yourself in Australia (statements, job offers, etc)
• Australian police certificate (if applicable)
• Overseas police certificates (if applicable)
• Military service records or discharge papers (if applicable)
• Partner identity and relationship documents (marriage certificate, de facto evidence)
• Documents for dependent children (birth certificates, adoption papers, dependency forms)
• Documents for dependants over 18 (identity, character documents, proof of dependency)
• Proof of functional English or statement of intent to pay second visa charge for each applicant lacking functional English
• Australian Values Statement (age 18+)
