Documents

• Identity documents (eg. passport photo/details; national identity card if available; proof of name change)
• Statement explaining absence of travel or identity documents (if not available)
• Proof of humanitarian circumstances (UNHCR registration if applicable, detailed statement about why applicant left home country)
• Certified copies of visas or residence permits held
• Certified copies of marriage certificates or relationship registrations (applicant and included persons)
• Photograph (scanned, of the whole face against a plain background)
• Military service records or discharge papers (if served in armed forces)
• Form 956A or Form 956, if nominating someone to receive correspondence or provide immigration assistance
• Partner documents (identity, character, relationship documents, photograph, marriage certificate or proof of 12-month de facto relationship)
• Documents for dependants under 18 (identity, proof of relationship, visas/residence permits, marriage/relationship registration, character, photograph)
• Parental responsibility documents (court order or consent form 1229/statutory declaration, signature and photo ID of consenting person; Form 1257 if child not staying with relative/guardian)
• Documents for dependants over 18 (identity, relationship, character, visas/residence permits, photograph)
• Proof of dependency (financial, physical, or psychological)
• Translations of all non-English documents (with required translator information)
