Documents

• identity documents (passport, national identity card, proof of change of name)
• documents proving you or your parent was born in a participating country (personal details page of passport, birth certificate)
• marriage, divorce or death certificates, separation documents, statutory declarations (relationship)
• proof partner or dependent children not included (reason)
• Australian police certificate (if spent 12+ months in Australia since age 16)
• overseas police certificate (if spent 12+ months in country since age 16 in last 10 years)
• military service records or discharge papers (if served)
• partner's identity and relationship documents (marriage certificate, joint bank statements, leases etc)
• evidence of relationship registration (if de facto)
• evidence for dependants under 18 (birth certificate, adoption paper, parental responsibility documents)
• evidence for dependants over 18 (identity, proof of relationship, Form 47A, character/marriage/relationship docs)
• evidence of English language proficiency or PEV English Language Undertaking form (Form 1566)
• Form 1229 Consent form for child under 18 (if required)
• court order/adoption papers if applicable
