Documents

• Pages of the child's current passport showing photo, personal details, and passport issue and expiry dates
• National identity card, if they have one
• Proof of change of name
• Certified copy of current and any past passports or travel documents
• Photographs (4 recent photos, 45mm x 35mm)
• Child's adoption papers or adoption compliance certificate
• Evidence from central authority that the child can leave the country for adoption (if required)
• Letter supporting the adoption from central authority (if applicable)
• For expatriate adoptions: evidence of adoptive parent's residence, intention, full and permanent parental rights, and adherence to local law
• Police certificate from every country lived in for 12+ months since age 16 (for children over 16)
• Military service records or discharge papers (if applicable)
• Form 80 for character assessment (for children over 16)
• Form 47CH Application for migration to Australia by a child
• Form 40CH Sponsorship for a child to migrate to Australia
• Certified identity and travel documents for dependants under 18
• Birth or marriage certificate proving relationship for dependants under 18
• Certified translations of all non-English documents
