Documents

• Identity documents including passport pages and national ID card if held
• Proof of change of name, if applicable
• Certified copies of adoption papers or adoption compliance certificate, if adopted
• 4 recent passport-size photos of the child and any dependent child included in application
• Certified copy of the passport or travel document to be used for entry to Australia and any previously held passports
• Evidence of full-time enrollment and participation for applicants aged 18 to 25 years and studying
• Statement explaining any gap over 6 months between secondary school and further studies, for 18-25 year applicants
• Medical evidence confirming disability for applicants aged 18 or older with a disability
• Evidence of relationship to the parent (birth certificate, marriage certificate, adoption certificate, family book, or family status certificate)
• Evidence of financial dependency for applicants aged 18 or older (bank statements, money transfers, rent receipts)
• Identity, travel, and proof of relationship documents for dependents under 18
• Consent documents for applicants under 18: Form 1229 or statutory declaration plus ID document, OR Australian court order, OR confirmation that law allows departure
• Police certificates for applicants aged 16 or over from countries lived in 12+ months since turning 16
• Military service records or discharge papers if served in armed forces
• Form 80 for character assessment (for applicants over 16)
