Documents

• Application questionnaire
• Foreign state issued valid passport and a copy
• Six (6) color photos (35 x 45 mm)
• Birth certificate and copy
• Marriage certificate and copy (if there has been a change in the last name)
• Passport of spouse and Armenian translation (if there has been a change in the last name)
• Birth certificate of child and copy, or Armenian passport of child/parent and copy, or other proof of Armenian citizenship (if applying under clause 4)
• Proof of Armenian descent (one of: certificate of baptism with annotation of Armenian descent, official document from foreign state with Armenian descent, birth/ID document of parent/grandparent/sibling noting Armenian ancestry)
• Documents proving kinship with person of Armenian descent, if required
• Non-citizenship certificate of stateless person and copy (if applicable)
• Convention Travel Document of refugee (if applicable)
• Proof of residence status or lawful residency and copy (if applicable)
• Receipt for payment of required state filing fee
• Any additional documents as requested
