- Passport or travel document and notary-certified Armenian translation
- 6 color photos (35x45 mm)
- Document(s) confirming Armenian descent (see sub-items a-d)
- Parents’ passport or the passport of legal representative (for underaged)
- Passport of authorized person (if applicable)
- Receipt confirming payment of state duty (no duty for under 18)
