  • Visa application form duly filled out electronically, printed, dated, and signed
  • A recent identity photo (35mm x 45mm)
  • Passport valid for at least 6 months and photocopy of first two pages
  • Proof of residence in the Netherlands for the applicant (rental contract or utility bill or BRP)
  • Copy of the employment contract
  • Provisional work authorization prior to the work permit issued by competent employment services
  • Commitment by the employer organization in Algeria to repatriate the worker at the end or early termination of contract, endorsed by competent employment services
  • A travel insurance certificate covering Algeria, min. 30,000 euros medical/repatriation, valid for visa period
