- Valid passport (at least 6 months)
- Completed visa application form (in duplicate)
- Photocopy of passport
- Two recent identity photos
- Travel insurance and repatriation assistance certificate
- Employment contract or assistance/service provision contract
- Provisional Authorization for Temporary Work (APTT) if employment contract
- Commitment to repatriate foreign worker at end of employment (when applicable)
