- Copy of a US Driving License or Identification Card of the Libyan spouse
- Statement of consent from the Libyan spouse
- Marriage Certificate (registered at the Consulate)
- Completed application form filled-out with black ink or printed
- One recent passport-size (2x2 inch) color photograph with a white background
- Prepaid envelope with tracking number and completed return address label (FedEx, UPS, or USPS)
