Documents

  • Long term visa application form and one recent passport size picture
  • Passport (valid at least 90 days beyond the expiration date of the visa) and photocopy of the main page
  • Photocopy of student’s ID and driver’s license (or state ID)
  • Letter from the University in Italy confirming enrolment, program dates, duration, weekly hours, and accommodation address
  • Letter from the University in United States specifying enrollment and University Study Abroad Program in Italy details
  • Personal bank letter or bank statement showing required funds
  • Personal credit card statement indicating credit limit (if relevant)
  • Letter regarding scholarship/financial aid if granted
  • Schengen medical insurance covering evacuation and repatriation (for at least 15 days)
  • Personal Money Order addressed to Consulate General of Italy for visa fee
