- A photocopy of the passport or equivalent document with visa, if required
- A document certifying the family relationship with the Union citizen issued by the municipality of residence or by consular representations of the country of origin
- EU citizen’s receipt of application for registration in the resident population register
- Four passport-sized photographs
- If the applicant is the partner in a stable relationship, official proof of the existence of the stable relationship
