- Visa Application Form
- Recent passport-size photograph
- Valid passport plus 1 copy of photograph and expiration page
- For Non-US citizens: original and copy of Permanent Residence Card or other US immigration document allowing re-entry
- Copy of Driver’s License or State ID as proof of residence (or other proof of current address)
- Copy of the Nulla Osta from Italy
- Confirmed flight reservation
- Visa fee (Money Order or cashier’s check)
- Return envelope (pre-paid, self-addressed, trackable)
- Consent to send by mail (form)
- Certification of enrollment in Business Registry of the firm, which must have been operating for at least three years
- Statement from the legal representative of the firm ensuring compensation above the legal minimum in Italy for exemption from healthcare expenses
- Copy of latest tax return or budget proving sufficient earnings of the firm
- Copy of the declaration of responsibility already sent by the Italian contract provider/legal representative to the Direzione Provinciale del Lavoro – Servizio Ispezione del Lavoro
- Availability of suitable lodgings (lease, ownership deed, or declaration by resident)
- Nulla osta issued within previous 90 days by competent Questura in the city of intended residence
