  • Child's valid passport
  • Passport photo of the child (≤6 months old) or photograph retrieval code
  • Evidence of income or financial resources
  • Birth certificate with information on child's parents
  • Document on the custody of the child
  • Consent given by the other guardian (if applicable, confirmed by a notary public or another authority)
  • Supplement form for parent or guardian, LIITE_LAPSI (if parents/guardians not both moving to Finland)
  • Document showing legal stay in country of application
  • Certificate of establishment of paternity (if born out of wedlock)
  • Decision on adoption (if adopted)
  • Divorce certificate (if parents are divorced)
  • Death certificate (if parent(s) have died)
  • Colour copies of child's and guardian's passport data and notes pages (if applicable)
  • Consent from guardian’s spouse in Finland (if applicable)
  • Form MP_1 (if child already in Finland and applies for first RP)
