- Proof of financial means (original bank statements; declaration from supporting parents with passport copy if relevant)
- Health plan and accident insurance valid for the entire period of stay, covering hospital expenses and repatriation
- Copy of birth certificate stating parents' full names
- Flight itinerary stating expected arrival date in Brazil
- Attached passport-size color photograph against white background, affixed to receipt and signed
