- Passport: signed, valid, with at least one blank visa page
- Visa Application Form receipt with bar code
- One 2 x 2 inch passport photo, front view, white background
- FBI Background Check issued within the last 3 months
- Copy of birth certificate
- USPS money order for the application fee (USD 290 for US passport holders, USD 100 for others)
- For foreign residents: Copy of the US Alien Resident Card and/or a Valid Visa
