The eVisa terms state the eVisa is valid for one year from issuance, for multiple entries unless stated otherwise, with a maximum stay of 3 months. However, the portal's visa data for US nationals lists a single-entry option (validity 90 days) and a multiple-entry option (validity 365 days), each with a stay duration of 30 days.

What important funds, accommodation, invitation, insurance, onward-travel or passport evidence may be required?

The passport must be valid for at least 6 months at the time of entry, the traveller must arrive with the same passport used to apply, carry a copy of the eVisa on first arrival, and be able to support themselves (and any dependants) during the visit. Entry remains subject to meeting the conditions on arrival and to the discretion of an Immigration Agent. The official source does not state accommodation, insurance or onward-travel evidence.

Where/how is permission obtained when required, and under what important circumstances can a stay be extended or changed?

Applications are made online through the official eVisa portal at evisa.sl. If the application is accepted, the eVisa is sent as an email attachment; the portal's FAQ also states that once the application has been approved, the visa is issued upon arrival at the airport or border. The traveller must carry a copy of the eVisa at the time of first arrival. A granted eVisa cannot be extended, and if it expires before entry a new application and a new application fee are required. The official source does not state any circumstances in which the stay can be changed to another status.
