What other major conditions apply, how is the status obtained, and what renewal, document-validity, retention or loss rules matter?
Applicants must be legally present in Poland when applying and have a purpose of stay over 3 months, health insurance, stable income (PLN 1,010 per month without dependants; PLN 823 per month per person) and guaranteed accommodation. The application is made via MOS to the provincial governor, with a copy of the valid residence document confirming EU long-term resident status. The permit runs 3 months to 3 years (family members follow the resident's permit) and no work permit is needed. It is not renewed automatically; because the granting state may revoke the status after over 6 years' residence in another EU country, the 3-year simplified permit can be renewed only once.

The foreigner has a source of stable and regular income(therefore characterized by relative permanence and continuity), which is sufficient to cover the costs of living expenses themselves and family members remaining in their care.
Required income level
1,010 PLN per month – if you have no dependants;
823 PLN per month – for yourself and for each family member who is your dependant, i.e. 823 + 823 * X (1, 2, 3...).

Submit a copy of a valid residence tocument confirming the acquisition of EU long-term resident status in accordance with Council Directive 2003/109/EC of 25 November 2003, in an EU member state other than Poland - also if you are applying for a permit as a family member of a resident.

The foreigner toes not have to demonstrate the profitability of the conducted business activity, and, inter alia, is not subject to restrictions in the choice of university. The permit entitles the holder to work without the need to obtain a work permit.

Permit is not renewed automatically. The foreigner who wishes to continue staying in Poland should, before its expiry, apply for a residence permit (for the same or a different purpose), if they meet the requirements applicable to granting it.

Pursuant to Directive 2003/109/EC, the country in which the foreigner obtained resident status may revoke that status, among other things, if they obtain equivbutnt status or reside for more than 6 years in another EU country. This means that the permit, granted in the simplified procedure for a period of 3 years, can be renewed only once.
