For the relevant traveller scope, is a visa, electronic authorisation, visa on arrival or no advance visa normally required?

A visitor visa may be issued on arrival to citizens of countries with which the Republic has diplomatic ties, as listed by the Ministry of Foreign Affairs from time to time. Except as provided under §5 (exemptions) and §6 (visa on arrival), citizens of all other countries must apply for a visa before entering.

What important funds, accommodation, invitation, insurance, onward-travel or passport evidence may be required?

Before issuing a visitor visa on arrival, the Director must be satisfied that the person has a return ticket, provides an address while in the Republic, and any other information the Director may require. A visa applicant must carry a passport expiring no less than six months from the expiry date (as worded).

Where/how is permission obtained when required, and under what important circumstances can a stay be extended or changed?

On arrival, a person eligible for a visitor visa upon arrival is issued with a corresponding permit. A visitor visa cannot be renewed or extended, and a visitor-visa holder may not change status while in the Republic.
