Where/how is permission obtained when required, and under what important circumstances can a stay be extended or changed?

A visitor using a valid exemption does not apply for a prior visa; the visitor pass is issued on arrival. Extension is discretionary for special circumstances such as illness, accident or war, not routine renewal. The extension application is made in person with passport, IMM.55, supporting proof and a confirmed homebound ticket or a third-country ticket and visa where needed.
