Where/how is permission obtained when required, and under what important circumstances can a stay be extended or changed?

A visitor extends their stay by submitting an application for an extension; an approved extension is a stamp in the passport granting extra time. The application should be submitted before the landing time expires; failure to do so puts the person in breach of Immigration laws and incurs penalties. Documents: passport valid no less than 6 months to expiry, return ticket, letter stating reason for extension, entry visa (if applicable) and the required fee.
