For the relevant traveller scope, is a visa, electronic authorisation, visa on arrival or no advance visa normally required?

A Guatemalan visa is only required for foreigners classified in nationality categories "B" and "C"; category "B" nationals already holding a valid US, Canadian or Schengen visa do not need a separate Guatemalan visa, while category "C" nationals must obtain one regardless, either at a Guatemalan consular mission abroad or, in-country, before the Guatemalan Institute of Migration (IGM).

What important funds, accommodation, invitation, insurance, onward-travel or passport evidence may be required?

A passport must have a minimum validity of six months to apply for a Guatemalan visa. Category "B" applicants (applying at a Guatemalan consular mission abroad) need the visa application form, a valid passport, two photographs, a reservation for air/land/sea travel, proof of income or bank statements from the last three months, an international credit card and proof of payment; category "C" applicants (applying before the IGM) need the application form, a valid passport, certification of the passport's validity, a clean criminal/police record, a transport ticket or a Guatemalan guarantor, and proof of payment.

Where/how is permission obtained when required, and under what important circumstances can a stay be extended or changed?

In Guatemala, visas are issued by the Sub-Directorate of Immigration of the Guatemalan Institute of Migration; abroad, by the Ministry of Foreign Affairs through consular missions. A separate extension route exists for staying longer; the official source does not state the extension conditions.
