For the relevant traveller scope, is a visa, electronic authorisation, visa on arrival or no advance visa normally required?

Cambodia issues a single-entry Tourist e-Visa (Visa T), applied for in advance through the official evisa.gov.kh portal. Nationals of Laos, Malaysia, the Philippines, Singapore, Vietnam, Thailand, Indonesia, Brunei Darussalam and Myanmar are visa-exempt and do not need this e-Visa; all other nationalities normally need to obtain it.

What important funds, accommodation, invitation, insurance, onward-travel or passport evidence may be required?

The applicant needs a passport valid for more than six months at the time of entry, a recent digital passport-size photo (JPEG or PNG), and a valid Visa/MasterCard credit card to pay the application fee.
