Which travellers require a transit visa/permission and which officially documented exemptions are important?

If permission is required, what permission is it, where/how is it obtained, and what core onward-travel evidence is required?

Visas are granted by the competent authority of the Ministry of Foreign Affairs. The general requirements are the sworn-statement application form obtained from the websites of the Dirección General de Migración or the Directorate General of Consular Affairs or at the consulate, a passport valid for at least six months, and a yellow fever and/or other health certificate where applicable; the specific requirement is tickets showing a third country as final destination. Transitory stay for transit is processed at the central office, departmental administrations or enabled border migration posts.
