Does the applicant need a job offer, employer, sponsor, transfer relationship or other employer involvement? What is important about that employer relationship?

The applicant must be either nominated by an Australian state or territory government agency or sponsored by an eligible relative. A relative sponsor must be 18 years old or older, usually resident in a designated area of Australia, an Australian citizen, Australian permanent resident or eligible New Zealand citizen, and the applicant's eligible relative or their partner's eligible relative if the partner is also applying; the sponsorship must be approved before the visa can be granted. The official source does not state any requirement for a job offer or employer.

What are the few decisive professional characteristics of the pathway: occupation, qualifications, experience, salary, skill level, shortage status, labour-market condition or equivalent?

This is a points-tested skilled visa: the applicant must have an occupation on the combined list of eligible skilled occupations for the subclass 491 visa and a suitable skills assessment for it, obtained in the 3 years before the date of invitation (a suitable assessment obtained within the 60 day invitation period is also accepted); have at least Competent English at the time of invitation; be aged under 45 to be invited; and score at least 65 points to be invited. The holder must live, work and study in a designated regional area of Australia. The official source does not state a salary requirement.

What work does the permission allow? Is the holder tied to an employer, occupation or sector? Can family accompany where clearly established?

How long is the permission normally granted for, how is it renewed, and what longer-term residence direction exists if officially stated?

This is a temporary visa allowing a stay of 5 years from the date of grant, and it cannot be extended. If eligible, the holder can apply for permanent residency through the Permanent Residence (Skilled Regional) visa (subclass 191); the holder cannot apply for other permanent visas until the subclass 491 visa has been held for 3 years.
